Legacy Programs is founded and operated by family members with established careers in mental health and community service. The foundation of our programs is the relationships we build with our participants as “Coaches”. Coaches hold a place of respect and authority, but still feel reachable enough for individuals to share their goals and to view their coach as a role model. The program is designed with the following goals in mind:

The intent of this program is to assist students in accessing their communities and engaging in community-integrated activities with typical individuals in natural settings. This program’s intent is to create relationships to coach our participants in living their best life possible. In doing this, we hope to give them the skills they need to live as independently as possible. Through these programs a variety of skills include, but not limited to, communicating with others, making personal choices, eating independently, personal hygiene, shopping, domestic skills, developing hobbies, reinforcing interests, and development of basic work skills. Each person’s program will be individually designed to meet their personal needs, skills, and goal attainment. These skills will be emphasized through a variety of activities designed and coordinated by a trained staff.
Another important aspect of the integration program will be community service projects. This part of the program will teach our participants the importance of giving back and being a part of their communities. The program will include the development individualized programs for each participant, specific goal setting, planning activities, and working with volunteers. Volunteers will include family members of the participants themselves. Overall, it is crucial that the program plan is person-centered and is developed with active participation from the participants.
Participants regularly attend community outings, including trips to museums, parks, community events, and entertainment and social activities. Our programs assist individuals in achieving personal goals and accessing services, via modeling behavior by our coaching staff.
To help individuals successfully hold jobs of their choice within their communities, we provide a full range of training and support services. Our job developers analyze the interests and capabilities of each person, suggesting technical or social-skills training that enables a successful job experience—for both the employee and the employer.
Program participants also can be matched with a job coach who provides customized on-site support, including transportation, job training, and oversight. We also focus on building partnerships with community employers. These partnerships expand opportunities for those we serve and help businesses grow by hiring workers who are well-trained and motivated to succeed.

The Person-Centered Planning program is designed to empower students by facilitating their integration into the community and helping them engage in activities alongside their peers in natural settings. Our goal is to build meaningful relationships and provide coaching that enables participants to live their best lives independently.
The program focuses on developing essential life skills such as communication, personal choice-making, hygiene, independent living, and basic work abilities. Each plan is tailored to the individual’s unique needs, ensuring personalized support and growth through activities led by trained staff.
The Legacy Lifestyle Program (“LLP”) was created to support our participants pursue healthier lifestyle choices which includes nutrition and diet, weight management, physical activity and good hygiene practices. Our Direct Support Professionals are referred to as “Coaches” and will support our participants to take action towards achieving their individual health goals. The LLP is designed to help individuals with disabilities achieve their full potential. This program provides a safe and supportive environment for our participants to build self-confidence and gain valuable lifestyle habits. Participants will receive personalized training, mentorship, and ongoing support to ensure their success. The program also offers guidance on maintaining a healthy work-life balance and navigating social situations in an integrated environment. With this approach, individuals can overcome barriers and achieve their personal goals with confidence.
If transportation provision is required due to behavioral, safety, or medical support needs; or the participants need assistance with transportation logistics, Legacy Programs can provide pick-up and drop-off services at home, or another community location and support the participant to learn how to use public transportation, which would include accompanying a participant to register for Access and Dial a Ride or similar service, as well as using the local bus/public transportation.
